Community Palliative Care Nurses visiting patients in their homes

The team of nurses provide an outreach service to patients, relatives, carers and professional colleagues working with the GP and district nursing teams and visiting individuals in their homes in the Salisbury area and surrounding rural villages. The team work alongside the patient's GP and district nurse ensuring a high level of care for the patient in their own home. The specially trained team advise on symptom control and provide emotional support for the patient and family within their home. Patients are discharged if their situation stabilises and they are managing their illness.
