Hospital Palliative Care Nurses visiting patients in Hospital

The team of nurses provides an outreach service to adult wards in Salisbury District Hospital. Hospital staff refer for advice on symptom control and emotional support for the individual patient and their families/carers whilst in hospital. The team also provides education and support to the health care professionals involved. When appropriate, the hospital team ensures continuity of palliative care on discharge by liaising with community palliative care teams.
